APPENDIX C

INITIAL PARTNERSHIP STRUCTURE

View Royal Primary Care Clinic Society

The View Royal Primary Care Clinic Society shall:

• Operate the clinic;
• Manage funding and financial administration;
• Lease and manage clinic facilities;
• Maintain insurance and corporate compliance;
• Report annually to funding partners and the community;
• Ensure municipal funding is used to advance the purposes of the Society.

The Town of View Royal shall:

• Appoint two directors to the Board;
• Provide operating funding of approximately $140,000 per year, payable monthly;
• Provide annual facility lease support as established by agreement;
• Receive annual audited financial statements and performance reports.

Aroga Lifestyle Medicine shall:

• Appoint two directors to the Board;
• Supply physicians, nurse practitioners, nurses, medical office assistants, reception staff, and clinical management services;
• Support patient attachment and ongoing clinical operations;
• Enter into a Clinical Services Agreement with the Society.
