IF THIS APPLICATION IS TO RENAME A MUNICIPAL ASSET, PLEASE ATTACH THE COMPLETED PETITION FORM (SEE SCHEDULE “C”).
Application Fees:
The appropriate fees must be submitted at the time of a renaming application as described in section 3 of the Policy. There is no fee to either name a municipal asset or to suggest a name or names for the Schedule “B” list.
I acknowledge the responsibility to pay for any and all costs for renaming a municipal asset, including but not limited to all municipal costs as well as private signage that will be impacted if the application is approved and all required address changes.
